What is Premier Pain Treatment Institute?
Premier Pain Treatment Institute is a leading pain management practice dedicated to providing innovative treatments for acute and chronic pain. With four locations in Middletown, Mt. Orab, Hillsboro, and Loveland, Ohio, we specialize in personalized care to help our patients find relief and improve their daily lives.

How do I schedule an appointment?
You can schedule an appointment by calling any of our four locations or by filling out the appointment request form on our website. Our team will contact you promptly to confirm the details.
Do I need a referral to be seen?
A referral is required in order to schedule your first appointment, and can be obtained by contacting your primary care physician, physical therapist, chiropractor, or even your current pain management provider. You're also welcome to contact us directly and we’ll guide you through the process.
What should I bring to my first appointment?
Please bring the following:
A valid photo ID
Insurance card(s)
A list of current medications
Any relevant medical records or imaging results
How long will my first visit take?
Your first visit typically lasts about 30 to 45 minutes. This allows our providers to thoroughly review your medical history, discuss your pain concerns, and create a personalized treatment plan.
Insurance and Billing
Do you accept insurance?
Yes! We accept most major insurance plans. For a full list of accepted insurances or to confirm your coverage, please contact our office at 513-454-7246.
What if I don't have insurance?
We offer self-pay options for patients without insurance. Our team can discuss pricing and payment plans to ensure you receive the care you need.
Will I know the cost of my treatment beforehand?
Absolutely. We provide transparent pricing and will review any costs with you prior to treatment. In certain cases where our receptionists are unable to provide detailed information, our billing company will be more than happy to assist you. Our goal is to avoid any surprises when it comes to your care.

Founded and led by Dr. Michael Danko, our team is made up of expertly-trained physicians, nurse practitioners, and physician associates. We also have a skilled support staff, including medical assistants, office managers, and a dedicated research team.
Do you perform surgeries?
Yes, we offer outpatient surgical procedures through Premier Surgical Institute in Loveland, Ohio — one of Cincinnati’s only physician-owned surgery centers. It’s equipped with two operating rooms, a procedure room, and a post-anesthesia care area.

Premier Surgical Institute (PSI) is our ambulatory surgery center located in Loveland, Ohio. It allows us to perform advanced procedures in a comfortable, cost-effective outpatient setting.
